WRITTEN PRESCRIPTIONS OR PRESCRIPTIONS BROUGHT FROM HOME
If you have a written prescription from your home physician or from one of our local physicians,
you may fax it from your hotel's front desk. Just let the hotel staff know that you wish to fax
your prescription to us and they should supply you with our Patient Information Fax Form.
You can also fill out this form online here. Simply fill out the form for the patient and fax the
form and the prescription to us. Once the fax is sent, be sure to contact us to verify pricing
and medication availability. We will then fill the prescription and deliver it to the front desk of
your hotel. Most hotels will the hold the medication at the front desk for you so you do not have
to be there for the delivery. Please note, some medications are not able to be handled in this
manner. We will contact you if this is the case.
Please note, not all prescription medications can be handled via fax. Please call us to verify
that your medication can be handled in this manner.

Please Note, we do not bill insurance plans. We provide itemized receipts and universal
claim forms for patient reimbursement. Patients with out-of-network insurance coverage may
submit the paperwork for reimbursement upon returning home from their trip.
